Here's the deal: we don't accept any insurance, and as a result you save money. Become a member of our practice for a low monthly fee, and get unlimited office visits, wholesale prices on prescription medication, your doctor's cell phone number, and a whole lot more.

How Are We Different?

We're a Direct Primary Care (DPC) practice. DPC is a unique approach to healthcare and our patients love it. So, how is it different?

Fewer patients

Did you know that most Primary Care Providers have between 2,500-3,500 patients? We limit our practice to just a few hundred patients. That means that your provider spends more time with you when you need them. Plus, when they are not actively seeing a patient, they are able to help coordinate your care and research your treatments.

No insurance

Willow Creek Direct Primary Care does not bill any insurance plans so you won’t have a middle-man in your primary care. Decisions about your healthcare will be made by you and your provider without the impersonal management that comes from third-party payers. Instead, patients pay a monthly membership fee which can save money and covers excellent treatment. Give it a try, and you’ll quickly see for yourself the value of this approach!

Meet Our Team


Dr. Ty Montgomery, DNP, ACHPN

Family Nurse Practitioner

Dr. Ty is a Family Nurse Practitioner who has been in practice for over 15 years in Northern Arizona, and in healthcare for over 35 years. After beginning his career as a firefighter/paramedic, he then went into nursing and worked in the Emergency Department and ICU before going back to school to become a Nurse Practitioner. He opened a private family practice in Sedona in 2006 which was then purchased by the local hospital in 2010. Following that he went to work with a national Hospitalist company and has worked extensively over the last 13 years in physical rehabilitation, geriatrics, and hospice medicine. He is now returning to a more traditional office based family practice as well as continuing to follow his patients in the rehabilitation and hospice settings. Dr. Ty holds Master's and Doctoral degrees in nursing, receiving his Doctor of Nursing Practice from Touro University Nevada. He holds national board certifications in both Family Medicine and Palliative Medicine. He relocated to Prescott in 2019 with his family, and is excited to be offering this new primary care model to our community.
